Kids Company

Thank you for registering for Kids' Company. We are thrilled you have chosen us to care for your child(ren). Kids Company provides care for children who are one year before Kindergarten, called KC WRAP thru 5th grade. We offer before and after school care at elementary buildings, wrap around care for children who are attending District 721 preschool, at Eagle View and CEC, along with non-school days and summer enrichment care.

Kids Company offers high quality programming with activities and curriculum based on the needs and interests of the children at the sites. Kids Company provides children with choice-based curriculum allowing the children to choose the activities they are most interested in. We strive to work with families to create an environment that encourages growth. The partnership created between parents, child and school is key to our success.

When completing your registration, please note that the primary contact person is the person that will receive the invoices and be responsible for payments. The primary contact person is also the only person that can make changes on the registration, such as schedule changes, emergency contacts, pick-up person, etc.

 I have received a copy of the Handbook for Families and understand the fees, payment policies, behavior expectations, procedures, and policies described therein
 I understand that I may not start with Kids’ Company until I have received confirmation from Kids’ Company.
 I agree to pay the following fees:
 Registration fee (non-refundable)
 Tuition payments: Payments will be accepted weekly for the upcoming week of service. Monthly statements will also go out on the 5th of month with tuition due on the 20th of each month for current month of care.
 Kids’ Company closes promptly at 6:00 p.m. When picking up my child, I will to come into the site, acknowledge to the Kids’ Company staff that I am taking my child, and sign my child out using the e-check out system. For the safety of my child, I understand that I will be charged a reminder fee of $5/day/child for disregarding the sign-in/out policies.
 I understand that late fees ($1/minute) begin at 6:01 p.m.
 I understand that I will be emailed a fee statement by the 5th of each month, with payment due on the 20th. I will be charged a late payment fee of $15 if my account becomes 5 days past due and that Kids’ Company services will be suspended, until payment is made in full, if my account becomes more than 30 days past due.
 I understand that if my bill is more than three months outstanding, the bill will be sent do a Board-approved collection agency.
 I understand that if my check is returned to the District for reasons of “Not Sufficient Fund” or “Account Closed,” I will be charged an additional $30/check.
Site Policies and Procedures
 For safety, I understand that I must provide Kids’ Company staff with the most current/accurate contact information at all times.
 At drop off, I will bring my child into the site, acknowledge to the Kids’ Company staff that I have brought him/her in, and sign my child in using the e-check in system. For the safety of my child, I understand that I will be charged a reminder fee of $5/day/child for disregarding the sign-in/out policies.
 If my child is attending the Kids’ Company afternoon session, I will place a Kids’ Company backpack tag on the outside of his/her backpack.
 If my child regularly attends Kids’ Company, but is going to be absent, I will inform my site’s Kids’ Company staff, by phone. Likewise, if my child attends infrequently, I will inform my Kids’ Company staff, by phone, that my child will be attending.
Serious Accident/Illness
 I give permission to Kids’ Company to take whatever emergency measures are judged necessary for the care and protection of my child while under the supervision of Kids’ Company. In case of a medical/life-threatening emergency, I understand that my child may be transported, at my expense, to the hospital by local emergency personnel as they deem necessary. It is understood that in some medical situations, the staff will need to contact the local emergency personnel before the parent and/or other adult acting on the parent’s behalf.
Sick child
 I will not send my child to Kids’ Company if he/she has vomited or had a fever greater than 100 degrees in the last 24 hours.
Medication
 Medication will be administered under limited circumstances, when prescribed or authorized by a physician. All medications must be given to the Kids’ Company site staff in the original prescription bottle and be accompanied by a completed Medical Administration form.
Fieldtrip & Transportation
 I give permission to Kids’ Company to take my child on supervised walking trips and/or bus trips. I will read and make sure my child understands the General Program Expectation, Field Trip Expectations, and Playground Expectations.
No babysitting policy
 Staff members at Kids’ Company are not permitted to babysit for families within our program. Parents can choose to waive this by providing written notice that you understand staff members are not being paid as New Prague District employees and therefore Community Education is not held liable for any accidents or injuries.
Both parents’ right to pick up
 Under the laws of the state of Minnesota, both parents have the right to pick up their child unless a court document restricts that right. Kids’ Company must have a copy of those documents to enforce the unauthorized pickup.
Publicity
 In the event that Kids’ Company children are included in any newspaper, radio, television, social media, or marketing materials, I give permission for my children to be included.
